
In a move aimed at enhancing the lives of individuals with disabilities and the elderly, the Aging and Adult Services Division (AASD) and Disability Services Division (DSD) have issued their latest weekly updates. The new directives, which took effect on November 1, 2023, call for the resumption of in-person visits for case management and care coordination alongside the implementation of a remote reassessment policy for long-term services and supports, according to an announcement from the divisions, as per Minnesota Department of Human Services.
Focusing on the need to streamline processes, the AASD and DSD have also introduced a Professional Statement of Need (PSN) form, effective January 9, 2024, to assist in the eligibility and enrollment procedures for Moving Home Minnesota (MHM). The form is intended for use by professionals and individuals who are looking for MHM transition services, which is a development expected to help ease the burden of paperwork for those involved, as stated in the Minnesota Department of Human Services.
Moreover, the Disability Services Division is reaching out to solicit applications for its Innovation Grants. Prioritizing innovative strategies that improve outcomes for the disabled, especially those receiving home and community-based services, DSD is offering grants up to $60,000 under their Innovation Small Grant Program and larger amounts for their Innovation Large Grant Program.
