The trouble began after Boston Inspectional Services Department (ISD) inspectors came across rat droppings in a common area used for food preparation, they also spotted holes in a wall that leads perilously straight to the basement; furthermore, a distressing sight was water dripping into a storage area where dry goods are kept, and, even sewage posed a concern as it was leaking from a bathroom in the basement into the building's common area, as Boston 25 News reported.

The ISD records posted on the "Mayors Food Court" website also disclosed unsettling details including storage failures such as products being kept on the floor and other hygiene breaches that occurred in November 2023; these included filth like black dust and a slime-ridden ice machine, troubling indeed was a sink coated with a build-up of dirt and dust.
