
In a move stirring both support and controversy, Governor Greg Abbott has taken a definitive step towards quantifying the cost of healthcare for illegal immigrants in Texas. On August 8th, Abbott issued an executive order mandating the collection and reporting of healthcare expenses involving undocumented individuals by public hospitals—an initiative he believes necessary due to what he perceives as the federal government's lax border control policies. The order stipulates a systematic data-gathering process, with hospitals being required to furnish quarterly reports starting from March 1, 2025.
As reported by the official website of the Governor of Texas, the Texas Health and Human Services Commission (HHSC) is now tasked with this extensive data aggregation effort. They will begin directing hospitals and additional identified providers to start collecting relevant financial data by November 1. According to Abbott, the policy change comes in response to "President Joe Biden and Vice President Kamala Harris' open border policies," which have allegedly resulted in Texas bearing the financial brunt for healthcare provided to illegal immigrants.
The Governor's stern position reflects his perspective on federal immigration policies and their economic impact on state resources. "Texans should not have to shoulder the burden of financially supporting medical care for illegal immigrants," Abbott said in a statement. In his view, the state should not be financially penalized for a situation it did not create, hence his push for the federal government to reimburse these expenditures.
Beyond the collection of costs, the directive also instructs hospitals to inform patients that their care will not be impacted by any response they provide in adherence to federal law—a caveat intended to uphold patient care irrespective of immigration status. Beginning on January 1, 2026, an annual report will be submitted to the Governor, the Lieutenant Governor, and the Speaker of the House to outline the previous year's costs associated with medical treatment provided to illegal immigrants. Abbott's long-term goal is to leverage these reports as a basis for seeking financial reimbursement from the federal government, holding it responsible for what he refers to as their costly and dangerous policies.
The implementation of this executive order will not likely be without its challenges, facing potential pushback from advocacy groups and legal hurdles. Nevertheless, the Governor seems poised to push forward with this collection mandate, setting the stage for a contentious debate on healthcare, immigration, and the intersection of state and federal responsibilities.
