According to OKC FOX, the Oklahoma Highway Patrol reported that the collision took place at approximately 8:49 p.m. on Saturday, as Smith's vehicle crashed into the rear of a Freightliner driven by 58-year-old Demetrious Denson. Denson, hailing from Lancaster, Texas, was unharmed despite his vehicle being slowed due to traffic congestion.

The investigation by the Oklahoma Highway Patrol has concluded that inattentive driving was the cause of the accident. Smith was trapped inside his vehicle for around an hour before he could be extricated by efforts of the Goldsby and Purcell Fire Departments. Despite their attempts, he was pronounced dead at the scene. KOCO reported that both Smith and Denson were wearing their seat belts at the time of the collision.

KFOR stated that delays affected traffic going northbound near Mile Marker 103, which is situated just south of the Riverwind Casino. Officials warned that the highway would be brought down to one lane while the crash was being worked on, further contributing to traffic delays.
