
Teachers Health Trust, Nevada's largest educator health plan, has undergone a dramatic transformation in its approach to bariatric surgery, shifting from explicit exclusions to comprehensive coverage in what represents one of the most significant policy reversals in local healthcare.
The 42-year-old self-funded, nonprofit health insurance plan for educators in Clark County now offers what it calls a comprehensive bariatric surgery program through its BARInet provider network, according to THT Health. This marks a stark departure from its previous stance on weight-loss procedures.
From Exclusion to Comprehensive Coverage
The change represents a complete about-face for the organization that once took a hard line against bariatric surgery coverage. Historical evidence shows THT previously sent members letters stating that "bariatric surgery is not a covered benefit under the Teachers Health Trust PPO Benefit Plan, and any complications incurred as a result of that non-covered surgery are also excluded," according to discussions on Bariatric Facts from members who faced coverage denials.
Today's program tells a different story entirely. THT's bariatric program is designed for members with a BMI of 40 or greater, or a BMI of 35 with comorbidities, requiring participants to meet with a THT BARInet Representative to discuss program details and commitments.
Program Requirements and Coverage
Eligible members must be covered for at least one year before beginning pre-operative requirements, as detailed by Teachers Health Trust. Approved procedures include Sleeve Gastrectomy, Duodenal Switch, and Gastric Bypass, though the program covers only primary bariatric procedures and excludes revision surgeries.
The program's financial framework reflects a middle-ground approach to cost-sharing. Members face a $5,000 co-pay for the surgery benefit, which can be paid using Flexible Savings Account or Health Savings Account funds, according to THT Health. Members remain responsible for coinsurance after deductible and applicable office visit co-pays.
Holistic Wellness Approach
THT's weight loss support extends beyond surgical options. The plan includes Wondr, a digital weight loss program offering "on-demand master classes" and community support, as reported by Teachers Health Trust.
The organization also provides one-on-one nutritional counseling, with success stories including a member who lost 18 pounds working with a registered dietitian between February and May 2023, according to THT's wellness program.
