A 78-year-old Miamisburg woman, Pamela Goins, died Tuesday after her SUV left the road on Wolf Creek Court in Carlisle and went down an embankment, slamming into a tree. Witnesses and a Carlisle police officer pulled Goins from the vehicle and began CPR before she was taken to Atrium Medical Center, where she later died. Authorities identified the driver as Goins and said the Ohio State Highway Patrol is investigating the crash.
The Ohio State Highway Patrol told Local12 that troopers from its Lebanon post responded to a 911 call shortly after 6 a.m. and found a 2023 Chevrolet Traverse off the roadway. According to the preliminary trooper report cited by Local12, the SUV first hit a mailbox, returned to the roadway and struck a parked car before traveling between two houses and down an embankment.
Witnesses at the scene and a Carlisle police officer helped remove Goins from the SUV and started life-saving measures until medics arrived, according to WHIO. She was taken to Atrium Medical Center in Middletown and later succumbed to her injuries, local reports say.

A preliminary investigation indicates the SUV was traveling westbound on Wolf Creek Court, west of Sunset Drive, when it veered off the left side of the road and hit a mailbox, then re-entered the roadway and struck a parked vehicle before leaving the road again and striking a tree, as reported by WCPO. Troopers and local emergency crews remained at the scene as the Ohio State Highway Patrol worked to reconstruct the sequence that led to the collision.
