The incident, which took place on May 29, 2022, involved O'Farrell veering into the oncoming lane and colliding with five motorcycles that were part of a Connecticut motorcycle club on routes 5 and 10 in Northfield, Massachusetts. Two children, aged 11 and 13, were also in the vehicle with O'Farrell, but they did not sustain injuries during the crash, as reported by MassLive. Judge John Agostini highlighted O'Farrell's substantial criminal record, indicating a history of indifference, at best, regarding compliance with the law.

In statements given to WPRI, prosecutors described the grim consequences for the victims, including loss of limbs, livelihood and the prospect of futures filled with "acute pain, economic uncertainty, and emotional trauma." Assistant District Attorney Matthew Thomas emphasized the life-altering impact on the victims due to O’Farrell's actions, stating, "We hope today’s guilty plea and sentence bring some measure of justice."

Witnesses at the scene reported that O’Farrell displayed signs of impairment, and during his guilty plea, he admitted to being under the influence of Suboxone at the time of the accident. Along with the aforementioned charges, O'Farrell also pleaded guilty to two counts of endangerment of a child under 14 by operating a motor vehicle under the influence of drugs, one count of negligent operation of a motor vehicle, two counts of assault and battery by means of a dangerous weapon, and one count of unlicensed operation of a motor vehicle, as per NBC Boston.
