
A father and two of his children died in a head-on collision in Yavapai County. Four others were hospitalized with serious injuries. The crash happened around 3 p.m. last Saturday near Williamson Valley Road and Levie Lane, northwest of Prescott. The Yavapai County Sheriff's Office said Jesse Forman, driving a Toyota 4-Runner from Chino Valley, was with his three children and their mother when his vehicle crossed double yellow lines and collided with a Jeep SUV, according to FOX 10 Phoenix.
Yavapai County Sheriff's deputies found a man and one child dead at the scene. Another child died on the way to the hospital. A third child is in critical condition at a Phoenix hospital, as detailed by 12News. Three adults were airlifted for treatment. A woman in her 60s and her adult son, who were in the Jeep, were taken to a hospital in Flagstaff with serious injuries. Officials have not specified the severity of the mother's injuries in the 4-Runner.
