Please verify that the e-mail address listed above for each investigator matches the e-mail address that you have received the e-mail survey from.”

The notices, often sent by HMS—the state’s official contractor—require members to confirm they live in Georgia. Delaying or ignoring the request could lead to a loss of coverage.
