
A late afternoon drive on a quiet Farmington back road turned deadly Wednesday when two SUVs collided at the rural intersection of County TK H and Forest View Road, leaving one person dead and another injured, officials said.
The crash, reported around 4:19 p.m. in the Town of Farmington in Washington County, drew multiple 911 calls and a large response from local volunteer firefighters and rescue squads. Deputies and first responders tried life-saving measures on one driver at the scene but were unable to revive that person. The intersection remained an active investigation site into the evening as authorities worked to reconstruct the crash.
What Officials Are Saying
According to a release from the Washington County Sheriff's Office, deputies were sent to the intersection after several 911 callers reported a serious two-vehicle collision. The Boltonville Fire Department, West Bend Rescue and other emergency crews also responded.
When deputies arrived, one SUV driver was unresponsive while the other remained conscious and alert, the sheriff's office said. Life-saving efforts were focused on the unresponsive driver, but those efforts did not succeed.
How the Crash Unfolded
Preliminary findings and witness accounts indicate a gray SUV was traveling east on County TK H and tried to turn north onto Forest View Road when a westbound blue SUV hit the gray vehicle on the passenger side, according to FOX6 News Milwaukee, which summarized the sheriff's office report.
Multiple callers flagged the crash to dispatchers, and responders worked on the gray SUV’s driver, who was unresponsive at the scene, authorities said. Officials have not released any findings related to speed, impairment or other possible contributing factors, citing the ongoing nature of the investigation.
Investigation Still Active
The Washington County Medical Examiner's Office assisted the sheriff's office at the site, officials said. No charges have been filed and the names of those involved have not been released.
Investigators are asking anyone who witnessed the crash or has dash-cam video from the area at the time to contact the Washington County Sheriff's Office non‑emergency line.
Rural Intersection Shuts Down Traffic
The collision happened in the Town of Farmington, a rural stretch of Washington County located east-northeast of West Bend. Emergency crews remained on scene as investigators documented the site, and traffic in the area was affected during the response. Authorities did not say when the roadway was expected to fully reopen.
