Cyclospora, a microscopic parasite with a very loud impact on your gut, has now been confirmed in Mecklenburg County as North Carolina joins a broader multi-state outbreak this summer. The bug typically triggers watery, sometimes severe diarrhea along with cramps, nausea, and fatigue, with symptoms usually kicking in about a week after exposure. Health officials are urging residents to be extra careful with fresh produce while investigators work to figure out where contaminated items slipped into the food chain.

Today, the North Carolina Department of Health and Human Services took to X to confirm cases in Mecklenburg County and to remind residents that “thorough washing of fruits, vegetables, berries, and fresh herbs” can help cut the risk, according to NCDHHS. The agency also urged anyone dealing with persistent diarrhea to contact a health care provider so labs can specifically test for Cyclospora.

How Many Cases Are We Talking About?

Recent local tallies show Mecklenburg County has logged dozens of infections in just a few weeks as statewide totals climb into the hundreds. By mid-July, Mecklenburg had at least 29 confirmed cases and North Carolina had recorded more than 500 cases overall, according to Axios Charlotte.

What Investigators Are Zeroing In On

At the national level, federal investigators have linked a separate multistate cluster to shredded iceberg lettuce tied to Taco Bell locations and to product from Taylor Farms de Mexico. The Food and Drug Administration says it is coordinating recalls and sample testing as part of that probe. Traceback and epidemiological work point to iceberg lettuce sourced from central Mexico, and Taylor Farms de Mexico has voluntarily pulled the implicated product from the market, according to the FDA.

State Response and Local Tracebacks

North Carolina health officials say the spike in in-state cases does not currently appear to be directly tied to the Taco Bell and Taylor Farms cluster. In interviews with patients, commonly named ingredients include parsley, cilantro, and lettuce. The department is collaborating with local health departments and the state agriculture agency to trace exposures and pinpoint any suppliers linked to North Carolina illnesses, according to NCDHHS.

How to Cut Your Risk

Public health guidance is fairly straightforward, if not especially glamorous: rinse fresh fruits and vegetables under running water, scrub firm produce with a clean brush, wash your hands before and after handling raw produce, and skip bagged or pre-cut salads for now. Food safety experts note that washing can lower but not completely remove Cyclospora from produce, and cooking is the most reliable way to kill the parasite, according to NC State Extension.

When to Call the Doctor

If you come down with persistent watery diarrhea, sharp abdominal cramps, or symptoms that drag on for more than a few days, health officials say it is time to call a medical provider and mention possible Cyclospora exposure so targeted stool tests can be ordered. The recommended treatment is the antibiotic combination trimethoprim-sulfamethoxazole (TMP-SMX), which typically clears the infection quickly once started, according to clinical guidance from the CDC.
