CMS records identify the provider at the Karl Road address, while a Laurel Health Care release said the company acquired the former Villa Angela Care Center in 2018 and renamed it The Laurels of Walden Park.
The sale appears to involve the facility's real estate, not necessarily a change in the company operating the nursing home.
