A wellness check on a suicidal patient turned into a fatal roadside shooting Tuesday afternoon near Keystone Heights, after the man pulled a hidden handgun on Clay County deputies during a traffic stop on State Road 21. Emergency personnel pronounced him dead at the scene once backup deputies were able to disarm him.
The chain of events began around 1:42 p.m. Tuesday when Clay Behavioral Health Center called the Clay County Sheriff's Office asking for a wellness check on a patient in his 30s who had expressed suicidal intentions after being taken off a medication, according to WOKV. Deputies located the man during a traffic stop near Gas Line Road, in the 7000 block, an area the Florida Times-Union also placed at the center of the encounter in its initial report on the shooting.
A Hidden Gun Changes Everything
The man initially told deputies his gun was at home, but when a deputy attempted a weapons check, he pulled a handgun from his pocket, according to WOKV's account of the incident. What followed was a roadside standoff in which the man repeatedly refused orders to drop the weapon before a deputy opened fire, the same report states.
Early advisories from the Clay County Sheriff's Office indicated the man had been transported to the hospital, and no deputies were injured, per the sheriff's advisory cited by the Times-Union. That status has since been updated: Clay County Undersheriff Ron Lendvay confirmed that emergency personnel pronounced the man dead at the scene after backup deputies disarmed him, WOKV reported, correcting the earlier hospitalization advisory.

The Legal Backdrop for Wellness Checks Florida's Mental Health Act, known as the Baker Act, allows officers to take people in a mental health crisis to a hospital against their wishes, according to WOKV. The deputies' response followed a behavioral health provider's call to the Sheriff's Office.
Because the encounter ended in a deputy-involved shooting, the case is being investigated by the Florida Department of Law Enforcement.
A Troubling Pattern in Clay County
Clay County has also had deputy-involved shootings in recent years.

The 2025 precedent is also notable: that April, two Clay County deputies, identified as Tyler Elias and Justin Mosley, shot a woman at 1717 County Road 220 after she pointed a handgun at them while they responded to a distress call about a possible suicide, leaving her critically injured, Hoodline reported at the time. Taken together, the incidents point to a recurring challenge for the sheriff's office: mental health calls that escalate into armed standoffs with little warning.
