Lake County General Health District (LCGHD) has teamed up with University Hospitals to conduct the 2025 Lake County Community Health Needs Assessment (CHNA), calling on county residents to offer their perspectives on many health-related issues, as reported by the City of Mentor’s website. Spotlighting concerns such as access to nutritious food, tobacco use, substance abuse, and mental health, the CHNA is looking to gather insights directly from the public through a survey that's available in English and Spanish.
Open to residents of Lake County who are 18 and older, the survey serves as a tool for LCGHD and University Hospitals to tap into the community's health pulse, aiming to draw a comprehensive picture of the prevailing health landscape. In contrast, the survey is open until at least Tuesday, December 31, and it takes less than 15 minutes to complete on various digital devices. Valuing confidentiality, the survey is voluntary and anonymous, encouraging honest participation without the fear of personal data being compromised. The survey details and access information are on LCGHD’s official website.
The CHNA's mission is to accurately gauge the current health demands of Lake County's populace and to pinpoint areas along with community health improvement strategies. It's a multi-faceted approach, integrating national and state data review, the community resident survey, and targeted community focus groups across the county to ensure that no health issue goes unnoticed.
According to Health Commissioner Ron H. Graham, “The population’s health is divided by the social determinants of health resulting in vastly different qualities of life. The resource-rich and well-connected are not only far more likely to receive care when they need it but they also start off with advantages that affect health like housing, employment, stress levels, food security, social capital and more.” Graham's statement underscores the importance of addressing health disparities, and crafting environments that foster well-being for everyone is part of the larger conversation that the CHNA aims to engage, as reflected in the Health Commissioner's comments captured by the City of Mentor’s announcement.
For those without internet access or who prefer physical documents, paper versions of the survey in both English and Spanish are obtainable from the Lake County General Health District office located at 5966 Heisley Road in Mentor and at chosen local partners’ establishments, should you need more details or want to assist in distributing the survey, LCGHD encourages contact via phone at 440.350.2543 to coordinate the collection of hard copies.
