According to a social media post from the San Diego Police Department, the 47-year-old woman was last seen at the nursing facility at 2828 Meadow Lark Dr. around 4 PM.

On April 15, 2025, Azucena Horta Cuentas was reported missing from The Shores Post-Acute Nursing Home located at 2828 Meadow Lark Dr in San Diego. She was last seen at the facility around 4PM on 4/15/25.
Described as a Hispanic female, standing at 5’5”, weighing approximately 230lbs, with neck length brown hair and brown eyes, Cuentas is also diabetic with a diminished mental capacity. This elevates the urgency of her situation, and the San Diego PD considers her at risk. The missing woman is believed to have been wearing a red sweatshirt adorned with black polka dots, grey sweatpants, and potentially, a medical-style bracelet containing her contact information.
