
A former Shelby County home healthcare agency operator has been indicted on charges of TennCare fraud and theft, authorities report. According to the Tennessee Bureau of Investigation, Elexis Braswell is alleged to have overbilled TennCare between $2,500 and $10,000, and is facing an identical charge for theft of property, as stated in a press release from Action News 5.
The TBI launched an inquiry after receiving a June 2022 fraud referral from TennCare's Office of Program Integrity, which indicated possible malfeasance by the owner of Tender Loving Care, also doing business as Elizabeth Home Care. It was found that, from July 2018 through August 2021, Braswell knowingly billed TennCare for services provided to patients who were concurrently admitted in local hospitals, leading to what the state agency describes as a loss of thousands of dollars for the State of Tennessee. Braswell's activities came to a head with her arrest on October 14th, 2025, followed by her indictment by a Shelby County Grand Jury in April 2025. The formal charges include one count each of TennCare Fraud $2,500-$10,000 and Theft of Property $2,500-$10,000. WREG reports that Braswell has been booked into the Shelby County Jail, her bail set at $20,000.
In detailing the charges against Braswell, authorities have highlighted the impact of fraudulent activities on state resources – funds that were meant, ostensibly, for the health and welfare of Tennessee residents.
