Lieutenant McCarthy experienced chest pain while she was on-duty at the St. Charles County Ambulance District Station 12 on November 21, which led to emergency surgery at Missouri Baptist Hospital. Despite the valiant efforts of the medical team, McCarthy suffered a significant setback and was later transferred to Barnes Hospital St. Louis, where she succumbed surrounded by family.

In a statement, as reported by the Governor's office, Governor Kehoe said, "Lieutenant Sarah McCarthy was a dedicated first responder who spent 23 years serving patients in need as a paramedic with the St. Charles County Ambulance District." He also highlighted the dedication of paramedics such as McCarthy, noting their commitment to protecting the safety and well-being of the communities they serve.

Flags were flown at half-staff at all government buildings in St. Charles County and at emergency medical service stations across Missouri from sunrise to sunset in honor of Lt. McCarthy. Officials also confirmed that McCarthy’s organs were donated through Mid-America Transplant, a decision that could benefit multiple recipients.
