Monday’s routine state health check did not go well for Butcher & Brew’s Halcyon outpost in Alpharetta. The restaurant logged a score of 67 out of 100 on the Feb. 9 inspection, a result that lands squarely in unsatisfactory territory. Inspectors cited eight violations, including several priority problems tied to increased risk of foodborne illness. Among the issues: a sanitizer dispenser at the three-compartment sink that was not working, a leaking pipe sending water onto the floor, and prepared herb butter held above the safe 41°F mark.
Inspection details
According to inspection records reviewed by The Georgia Sun, the evaluation took place at the Halcyon location listed at 1400 McFarland Pkwy and carries permit number FSP-058-000158. The inspector reported that staff were not taking food temperatures, parasite-destruction letters and signed employee health-reporting agreements were not on file, and the person in charge could not locate test strips.
The report notes that some problems were cleaned up on the spot, including knives stored between a prep cooler and a steam table and tongs and cloths hung on hot-water lines. Other violations, however, were left for the restaurant to address after the inspection wrapped.
What the grade means
Under Georgia’s food-service grading system, any score of 69 or lower is considered unsatisfactory and typically triggers required corrections and a reinspection, as outlined in state food-service rules. The scale assigns an A for scores of 90–100, B for 80–89, C for 70–79 and U (unsatisfactory) for 69 and below. Repeat violations can lead to extra point deductions and may prompt enforcement action. When priority items are left uncorrected, health officials can order follow-up inspections and, in more serious cases, move toward suspending or revoking a permit.
Where it sits and next steps

What to watch
Customers and employees who are concerned about the findings can keep an eye out for a posted reinspection or correction report from state health officials and may call the number listed in the inspection record for details. We will update this story if the restaurant issues a response or if a follow-up inspection is published by the Department of Public Health.
