
A UC San Diego–led analysis of a randomized clinical trial suggests that semaglutide, the GLP-1 ingredient behind drugs like Ozempic and Wegovy, slowed cellular markers of biological aging in adults with HIV and excess abdominal fat. The signal showed up across multiple DNA methylation “epigenetic clocks” over the trial’s 32 week treatment window. Researchers say the finding, if confirmed in larger studies, could point to broader geroprotective effects beyond weight loss.
The trial at a glance
According to Nature Communications, this result comes from a post hoc epigenetic analysis of a 32 week, randomized, double blind, placebo controlled phase 2b trial (ClinicalTrials.gov NCT04019197). The parent trial randomized 108 people with HIV associated lipohypertrophy, and paired blood samples at baseline and week 32 were available for 84 participants used in the epigenetic analysis, per the trial record on ClinicalTrials.gov.
What the researchers found
As reported by UC San Diego, semaglutide treatment was associated with a broad pattern of slower biological aging across several epigenetic clocks, including an approximately 9 percent reduction in the DunedinPACE measure of the pace of aging. The team also observed significant signals on PCGrimAge and on system specific clocks linked to inflammation and organ system health.
How the drug might work
Investigators say semaglutide’s metabolic effects, notably reductions in visceral fat and chronic inflammation, are the most plausible drivers of the epigenetic changes. Michael Corley, the study’s first author, told KPBS that people with HIV often show signs of accelerated aging and that improving metabolic stress could slow those processes.
Limits and next steps
The authors caution that the analysis was exploratory and relied on blood based epigenetic markers that serve as statistical predictors rather than direct measures of lifespan, so the results do not prove that semaglutide will extend life or “reverse” aging, according to Nature Communications. They call for larger, longer and mechanism focused trials to test whether the methylation changes translate into clinical benefits and whether similar effects appear in people without HIV.
