The Greater Hamilton Chamber of Commerce marked the occasion with an official groundbreaking ceremony for the Primary Health Solutions Lindenwald Health Center on August 6, 2026.
The project addresses a gap that local officials and residents have flagged for years: Lindenwald, Hamilton's largest neighborhood, has limited healthcare providers relative to its population, according to WKRC. The neighborhood sits within ZIP code 45015 and is home to roughly 12,100 residents with a median household income of $45,543, per NeighborhoodScout. That income level falls well below regional averages, a gap that has historically driven demand for subsidized, sliding-scale care in the area.
A Federally Qualified Health Center Model
Primary Health Solutions operates as a non-profit Federally Qualified Health Center across Southwest Ohio, serving more than 40,000 patients annually with primary medical, dental, pharmacy, and behavioral health services regardless of insurance status or ability to pay, according to the Health Resources and Services Administration. That FQHC designation requires the organization to deliver care through sliding fee scales based on household income, making the new Lindenwald site a direct extension of that mission into a neighborhood where affordability has been a persistent barrier.
A Butler County health needs assessment conducted by Scale Strategic Solutions identified Hamilton as a priority community facing high social vulnerability index scores, public transit barriers to regional medical centers, and local primary care provider shortages. Those transit gaps have often kept low-income residents from reaching centralized hospital systems for routine or preventive care, underscoring why a walk-in facility embedded directly in Lindenwald matters for the neighborhood's day-to-day health needs.
Part of a Broader Regional Shortage
The Lindenwald project also lands amid a statewide primary care crunch. State public health mapping from the Ohio Department of Health indicates that 57 of Ohio's 88 counties contain designated Health Professional Shortage Areas, where patient-to-physician ratios exceed 3,500 residents per primary care clinician. Butler County's local shortages are part of that broader pattern, and new community health infrastructure like the Lindenwald center represents one of the few levers available to close that gap at the neighborhood level.
Primary Health Solutions has already built a track record of neighborhood-embedded care elsewhere in Butler County. The organization established the nation's first fully integrated suburban school-based health center model in the county, placing permanent clinics inside public schools such as Fairfield and Hamilton City Schools to deliver medical, dental and vision care directly to students, according to the Fairfield City School District. City Partnership and New Technology

Community health centers like the new Lindenwald facility are designed to complement that kind of acute hospital capacity by handling outpatient and preventative care closer to where people live.
