As obtained by the Utah Department of Public Safety, the driver's door was "ripped open during the collision," which led to the driver being ejected and ultimately her death at the scene.

The local authorities have not released the name of the driver pending notification of the next of kin. The driver of the semi reportedly did not sustain any serious injuries. The crash resulted in traffic delays on SR-6 as emergency teams worked to clear the wreckage and conduct preliminary investigations.
