A man was left in critical condition Monday morning after a T-bone collision sent his vehicle careening off Highway 13 and down an embankment near the Elwood overpass, with bystanders finding him trapped and unresponsive before fire crews could pull him free. Approaching severe weather then forced a medical helicopter to abandon plans for an air transport, pushing its flight nurses and paramedics into the back of a ground ambulance instead to keep working on him during the drive to the hospital.

Witnesses began calling 911 around 10:30 a.m. on October 12, according to The Herald Journal. Gephardt Daily and KVNU News Talk report that Tremonton Fire was formally dispatched at 10:45 a.m. Initial reports described a serious collision in which a vehicle left the roadway and went down an embankment, and bystanders discovered a male occupant unresponsive and trapped inside, according to a statement from Tremonton Fire cited by Gephardt Daily. Fire crews extricated the patient from the wreckage, and Tremonton Fire paramedics began treating him on scene, per the same statement.

Weather Forces a Mid-Rescue Change of Plans

A medical helicopter responded to the scene and landed to help with the transport, but rapidly approaching severe weather meant it could not fly the patient out, per Tremonton Fire's account relayed by Gephardt Daily. Instead, the patient was transported in critical condition by ground ambulance, with the helicopter's medical team riding along to keep up critical care. Tremonton Fire dispatched Heavy Rescue 231 and Ambulance 231 to the scene, and flight nurses and paramedics from the AirLife Utah helicopter joined local paramedics inside that ambulance for the ride, as reported by KVNU News Talk.

That kind of improvisation has become easier since AirLife Utah secured official first-responder designation in Box Elder County in October 2024, a status that lets its flight crews land directly at emergency scenes and begin trauma care without waiting for ground units to first secure a landing zone, according to KSL TV. AirLife Utah is part of Air Methods, the largest U.S. air medical provider. Air Methods has regional helicopter bases in Brigham City, Orem, and Cedar City to cover trauma calls across Northern and Southern Utah.

A Known Trouble Spot on the Map

The crash occurred near the Elwood overpass on Highway 13.

Coordinated Response Across Several Agencies

Box Elder Communications was among the agencies involved in the response. Tremonton Fire publicly thanked bystanders, AirLife Utah, the Tremonton-Garland Police Department, the Utah Highway Patrol, the Box Elder County Sheriff's Office, and Box Elder Communications for their coordinated efforts, according to Gephardt Daily's report. The department also said its thoughts were with the patient and others affected by the crash.

The cause of Monday's collision remains under investigation, and Tremonton Fire said no additional details were available at the time of its statement.
